The practice does not tolerate violence, either physical or otherwise to any member of staff. If this occurs patients will be removed from the practice list.
Patients may also be removed from the list if there is an un-reconcilable breakdown in the doctor patient relationship which would affect patient care.
Every effort will be made at practice level to avoid removing patients from the list.
All patients will be notified by letter and details of the removal included.
